Hospice CNA Responsibilities:

• Provide hands-on personal care
• Perform simple procedures as an extension of therapy or nursing services
• Assist with ambulation or exercises according to the plan of care
• Assist with administering medications ordinarily self-administered
• Record pertinent information pertaining to the client’s care and strict adherence to the HHA care plan that is on the client’s chart
• Provide hands-on personal care including: Baths, oral hygiene, shampoos, assisting with/changing bed linens, assisting patients with dressing/undressing, skin care to prevent breakdown, assisting patients with toileting activities, and keeping patient’s living area clean and orderly, as appropriate
• Plan and prepare light meals/snacks according to specific dietary requirements of the client, and if necessary, feed the client
• Perform range of motion exercises and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

Hospice CNA Qualifications:

• CNA certification required
• High school diploma or equivalent preferred
• 3-5 years of hospice experience required.
